2. Create ONE PDF file that includes all of the documents in the order outlined below:
   1. Completed “CE Record Form”
   2. Ethics Completion Certificate
   3. MDHHS Training Certificates
   4. Other Training Certificates (if your training required MDHHS approval, please include the approval letter with the attendance certificate).
3. Use this link to double check you have all of the required documents and CEs.
